Dear Sir,

Dengue is a common mosquito borne infection that is still the global public health threat. There are many cases of fatalities due to dengue in outbreaks each year. The diagnosis of dengue can be problematic due to the fact that there are many similar infections.[1] At present, the use of dengue IgM/IgG serological test has become a useful tool for helping to definitively diagnose dengue.[2] However, there are also cases of dengue that visit to the physician before there is any evidence of immunity response to be diagnosed by serological test. The new NS1 antigen test is introduced for those cases.[3] However, the important concern of using any new test in clinical pathology is the cost. Here, the authors perform cost-utility analysis for adding NS1 antigen test to IgM/IgG test for dengue diagnosis. Data from 184 laboratory diagnoses of dengue in our laboratory during its outbreak in October 2015 was analyzed. In this assessment, the cost is the charge for the test in US (local currency, Thai Baht, 1 US dollar = 34 Thai Bath). The utility is the diagnostic rate of dengue. The cost utility in this work is the cost per dengue case diagnosis. The results are shown in [Table 1]. It seems that the cost per diagnosis for combining NS1 to IgM/IgG is higher. Although adding the NS1 antigen test to IgM/IgG test can help detect more cases, it costs more and seems based on medical economics concern.
